Blue Shield of Alabama Patient Health Snapshot

Many providers today access Blue Cross data on care gaps and other quality measures using a web portal and printed reports.  Now, providers can see that same data presented in their EMR workflow, without requiring a complex EMR integration or added expense.

Gaps

In 2022, the Smart Ribbon was introduced to 30 Alabama practices. Other primary care practices in Alabama saw the successes of those practices and quickly signed up to have the Smart Ribbon implemented in their practice. The Smart Ribbon is now in over 240 practices statewide. Benefits Include:
    • Automatically starts up with your EMR
    • Maintains user and patient context with the EMR
    • If the patient currently being viewed in the EMR is a Blue Cross and
    • Blue Shield of Alabama member (Blue Advantage® or commercial),
    • the Gaps app will visually indicate whether gaps exist
    • Surfaces gaps in care, risk capture, recent care, etc.
    • Dispute or Resolve diagnosis within the app
    • Clicking on the badge reveals the patient health snapshot info
    • See all attributed members in a list view, with filters, and have the
    • ability to print a summary list for easy reference
    • Ability to dispute conditions from the previous year
    • Annual Wellness Visit & AutoHR360 Tracking

    Auto HR360

    The Smart Ribbon and AutoHR360 app detects patient context from your EHR and pre-fills the form with the latest patient information from BCBSAL.The electronic form can be updated, saved and electronically submitted without having to print a paper form, scan it in and upload it. Benefits Include:

    • EMR-agnostic; no integration involved

    • Presents an electronic HR360 form that is pre-filled based on claims data
    • making it easy to complete and submit with no more printing, scanning, or

          The Gaps app automatically checks the patient being viewed in the EMR against the Blue Cross and Blue Shield of Alabama quality and risk measures. If the patient is not in the Blue Cross and Blue Shield of Alabama population, the icon checklist is blank. If the patient is in the Blue Cross and Blue Shield of Alabama and no care or coding gaps exist, the icon has a green check. If care or coding gaps exist, the icon checklist is orange and has a badge indicating the number of gaps. Clicking on the app icon reveals the gaps.
